If you are referring a patient for psychiatric care, our Psychiatry and Behavioral Sciences new patient packet can be filled out by the patient’s parents or guardians with your assistance.

For additional questions or to request a packet, please have the patient’s guardians call psychiatry at 202-729-3300.
The packet should then be returned to psychiatry via email, mail or fax (please see the packet for more information).
