If Your Child Needs Adult Services

As your child reaches age 21, you’ll need to think about what happens next.

If you live in Washington, D.C., our home care serves infants, children and young adults up to 21 years old. As your child reaches age 21, we will help you transition to services in the District of Columbia that provide care and support to adults with complex medical needs.

If you live in Maryland, your child can continue to get services from Health & Residential Services into adulthood. We provide care and support to children and adults of all ages who live in Maryland.

Our program works to connect young people who are moving to adult services with the resources they need.

If You No Longer Need Home Care Services

Our goal when you start home care services is to get you ready to care for your loved one on your own. The home care team will work with you to stop home care services when our staff and you together feel you are ready to safely care for your loved one.

Once it’s been decided that you are ready to stop services, we will create what’s called a “discharge plan.” This plan will have all the details you need about your family member’s care. It also will include community resources that can support you. We will make sure you have time to review everything and ask questions before you stop home care services.
