Valuable Tips to Help Expedite Your Forms Request 

As we look ahead to upcoming school registrations and summer camp seasons, we want to help make your child’s transition smooth and easy. Filling out health and physical forms correctly is an important step in keeping kids safe and ready for activity. 

To help us process your paperwork faster, please keep the following tips in mind: 

Complete the Parent/Guardian Section First

Before dropping forms off at the office or sending them through the portal, please make sure the parent/guardian section is fully completed and signed. We cannot legally or clinically validate the physician's section without having this information. Each form has sections clearly marked to guide you. 

List Medications and Preferences for Administration

Clear information about medications is important for your child’s safety while they are away from home. Sometimes medications may need to be given at camp or school, especially for sleepaway camps. Other times, a daily medicine can be given at home before a day camp or school day. Please be sure to share how and when medications should be given. This helps us coordinate your child’s care. Here are some key details of scheduled medications and medication types: 

  • Daily medications: any prescriptions your child takes every day, including dosages and specific timing. 
  • As-needed (PRN) medications: Share whether your child needs access to emergency or seasonal medications, such as epinephrine auto-injectors (EpiPens), rescue inhalers or allergy medications. 
  • Over-the-counter (OTC) preferences: Many forms require your permission for the school or camp nurse to give common medicines like ibuprofen or acetaminophen. 

Timely Submissions

Please remember that many forms require a physical exam within the last 12 months. Submitting these two to three weeks before the deadline gives our team enough time to review your child’s history and complete the forms carefully and correctly. 

Pro-Tip: Keep a Digital Copy

Once your forms are filled out and signed, take a quick photo or scan of the document for your own records. It is always helpful to have a digital copy on your phone in case a form is lost during the camp check-in process!
