Adhering to Report Deadlines

The EMSC Program requires that certain reports be submitted for all EMSC grantees, no matter the type of grant (Partnership, Targeted Issue, or Network Development Demonstration Projects). A brief description of each report is provided below along with their respective deadline.

  1. Successful applicants receiving grant funds will be required, within 120 days of the Notice of Grant Award (NGA), to register in the Health Resources and Services Administration (HRSA)’s Electronic Handbooks (EHBs) and electronically complete the program-specific data forms that appear in Appendix F of this guidance.

  2. Within 90 days of the Budget End Date, grantees must submit a Federal Financial Report (FFR) 425.  In previous years, this was the Financial Status Report (FSR) form.  The report should reflect cumulative reporting within the project period and must be submitted by logging into the EHB to complete the required forms.

  3. Within 90 days following the end of the project period, a final report is due (see Final Report Template), which shall contain information and data of interest to the EMSC Program, the Department of Health and Human Services, the U.S. Congress, and other states. A standardized final report form will be sent to all successful applicants.

  4. Within 90 days of issuance of the Notice of Grant Award, grantees must submit a Performance Report.  Grantees must log into the EHB to complete the Performance Report which includes the following:  financial forms, project abstract, grant summary, and performance measures.  Guidance on how to access these is now available.
