Follow My Heart

Heart Disease, Developed by Doctors at Children’s National
Other helpful resources
Follow My Heart is an online personal health record for patients with congenital heart disease. It is designed to help patients take an active role in managing their condition, particularly as they transition from pediatric to adult care.

Follow My Heart was developed by Linda Bradley Tiernan, MD, and Karen Kuehl, MD, pediatric cardiologists at Children’s National Medical Center and can be used by all patients with congenital heart disease. Each patient has a unique page that is protected and specific to his or her condition. The information is owned by the patient, who can control who has access to the information.

It provides patients an efficient way to communicate complicated health information to authorized healthcare providers. It can be accessed through any internet connection, so that patients and healthcare providers have easy access to the patient’s medical information.
