About Graduate Medical Education
The Medical Staff Office at Children’s National Medical Center is dedicated to serving the credentialing and privileging needs of our employed and community providers. Rotating Residents from other institutions are also credentialed by the Medical Staff Office.

Medical Staff Appointment Application Packet: (For MD, DO, DDS, PhD, NP, PA)
Dear Future Medical Staff Member: Please be advised, we accept only CAQH applications for medical staff credentialing. Kindly complete the profile document below and mail/ fax it to the Medical Staff Office. Upon receipt of this document, we will send you your CAQH provider ID. The UCD Reference guide is a step by step guide to walk you through the CAQH process. In addition we have included the following for your reference:
1) CAQH overview
2) License Information
3) Professional Licensing agency contact list
Rotating Residents: All Rotating Residents are requested to kindly submit a completed application packet at least a month prior to their start date.
